  Ear infections

Ear infections begin when a external agent inflames the ear canal and causes irritation, discharge and swelling. Ear infections are classified as Otitis media and otitis externa. Causes of ear infections can be a virus, cold or bacteria.

Symptoms

Some ear infections have no symptoms, but most do. Symptoms of an ear infection start with pain, severe pain in the ear. Other symptoms of an ear infection are fever, stuffy nose and sinuses. Ear infections commonly occur in children and infants, who may not be able to verbally identify what is wrong. Symptoms in children are ear tugging, sleeplessness, excessive crying, headache, ear drainage and troubles hearing.

Diagnosis

Diagnosis of ear infection can be made using a otoscope. An otoscope is a lighted instrument that allows doctors to view the inside of the ear. Fluid, mucus, inflammation are all indicators of an ear infection.

Other tests are used to determine how the ear infection is affecting hearing and general health of the auditory system. These tests are a tympanometry and an acoustic reflectometry. A tympanometry measures the eardrums movement in response to changes in air pressure in the ear. An acoustic reflectometry measures how sounds are bounced in the ear canal, providing a measurement of empty space versus fluid retention.

Causes

Fluid can build up inside the ear canal and behind the eardrum during a cold or sinus infection. This fluid can harbor bacteria which cause infection.

Some individuals are predisposed to ear infections due to genetic conditions such as Fragile-x syndrome and Down syndrome. Children can have narrow Eustachian tubes which connect the ears to the nose. Typically these tubes drain fluid and equalize pressure between the inside and the outside of the ear. Because the tubes are narrower in children than adults, fluid tends to accumulate easily and infection can occur.

Treatment

Ear infections are usually treated with antibiotics. Current recommendations are to wait 72 hours before considering antibiotics, because infections usually clear up on their own. Pain can be treated with over the counter medications. Make sure the proper dosage is used, especially when treating children.

Chronic ear infections can be treated by inserting a tube in the ear to allow better drainage of the fluids. This procedure is performed under general anesthesia with good recovery and results.

Ear infections that are chronic or long lasting can cause short term hearing loss, long term hearing loss and a ruptured ear drum. Short term hearing loss occurs because of the fluid build up and is equivalent to wearing ear plugs or being underwater and trying to hear someone on the surface. Short term hearing loss may or may not impact speech development. Long term hearing loss is caused by damage to the ear drums and the bones in the middle ear. This can damage hearing so that speech development and therefore reading can be adversely affected. A ruptured eardrum occurs when fluid and puss press through the eardrum. A ruptured eardrum can be very painful and will cause discharge, much like a stuffy nose from the ears. The discharge may contain blood as well. In most cases the ruptured eardrum provides immediate pain relief after the initial pain and heals on its own. An eardrum that ruptures often will need surgical repair.
